All fees are to be paid by municipal/government check or money order made out to "VA USA Boxing". No cash will be accepted by mail.
Non-Athletes must submit a backround check sheet (downloadable at our google group or by request) Backround check is valid for two years. All non-athletes must attend a certification clinic and pass written test before being able to perform thier duties ringside. This applies to all coaches and officials.
Athletes must submit two passport sized photos (above the shoulder) and provide proof of birth (copy of birth certificate) with their application for their passbook.
GENERAL INFORMATION
The membership year is a calendar year. It begins on January 1 and expires on December 31. Memberships are in effect from the date the application process is completed with the LBC: the application form is completed, the fee is paid, and all necessary documentation has been supplied. All memberships expire on December 31 and must be renewed on or before January 1 of the following year so there is no lapse in membership.
Athlete and non-athlete membership applications may be accepted beginning November 15 for the following year. Validation labels may be requested from USA Boxing headquarters as early as November 1 in order to process applications for the following year.
Validation labels are the key element in the registration system. When the Registration Chair places the label in the passbook and on the application, it signifies that all documentation has been received and the membership fee has been collected.
Validation labels come in sets of three, the A, B and C label, with a unique ten-digit number. Each year the membership labels are a different color and begin with a two-digit number signifying the year. For 2010, the labels will begin with “10”. 10 00208 99 5
The next five numbers are the individual member’s registration number. The next two are the LBC number, and the last digit is a check digit to ensure that the number is entered properly in our database. If we enter the number incorrectly, the system will flag the number as being incorrect and it must be corrected before being able to move on.
The cost for validation labels is $1.00 per set of three. There is a charge for these labels to help make sure that they are controlled by the LBC. Unused validation labels must be accounted for and returned to USA Boxing at the end of the year. If they are returned by March 15, the LBC will receive credit towards an equal number of new labels for the current year.
Because the validation labels are so important to the registration process, USA Boxing cannot accept registrations with handwritten numbers. The actual label must be in place, both on the membership application and in the athlete passbook.
ATHLETE REGISTRATION
The athlete membership application form is a two-ply form with a cover sheet that gives instructions on the information and documentation necessary to complete the registration. After processing the necessary information and collecting the membership fee, a validation label is placed on both copies of the application and in the passbook. The athlete membership fee for 2010 is $40.00. The top copy of the application is sent to USA Boxing and the second copy is kept for the LBC records.
Athletes must by 8 years old to be members of USA Boxing. This means they must have passed their 8th birthday, not that they will be 8 sometime during the year. The athlete must completely fill out and sign the athlete application. If they are under 18 years of age, the application must be signed by a parent or legal guardian.
New athletes must provide two passport size pictures and proof of age along with the membership application and fee before their application can be considered complete.
Renewing athletes need to produce their passbook, the completed membership application and the registration fee. Photos should be updated periodically.
The “A” validation label is to be placed on the application copy sent to USA Boxing.
The “B” label is placed on the athlete’s permanent record card kept at the LBC.
The “C” label is to be placed in the athlete’s passbook.
SOME THINGS TO REMEMBER:
1. The athlete must register using his/her own name. Ring names and nicknames are prohibited by USA Boxing rules (Article II, Section 102.4).
2. Both the Health Affidavit and the Release and Waiver must be signed by the athlete or, if the athlete is under 18 years of age, by his/her parent or legal guardian. Unsigned applications will be returned to the LBC to obtain the signatures. This is a requirement of the insurance company, and coverage may be denied if the applications are not properly signed.
3. The athlete’s date of birth must be provided on the application. Our database does not allow the entry of the membership without the birth date.
4. The date of birth must be verified. Acceptable documents are: a certified copy of the birth record from the department of vital statistics in the county of birth; baptismal, hospital, or tribal record from the state, county, or municipal Bureau of Vital Statistics or local jurisdiction of the place of birth. A current passport may also verify date of birth. The date of birth cannot be verified by a driver’s license.
MASTER ATHLETES (35 years of age and older)
New Master Athletes must provide the same information as all new athletes. They must also provide the results of their physical examination by their personal physician. They must receive a physical exam each year they are registered. Their examining physician must also sign the medical examination form on page 3 in the master passbook. The LBC must keep the examining physician’s report on file for each year that the master athlete registers.
Renewing Master Athletes must produce their passbook and the report of their physical exam. They should already have the physician’s signature on the medical examination form in the passbook.
PERMANENT RECORD CARDS
A permanent record card must be established for every athlete and kept in the LBC files. Information on each athlete is recorded on this card and in the athlete’s passbook. If the athlete should lose his passbook, his information is available from the permanent record card and not lost with the passbook. Each year the athlete registers, the B copy of the validation label is placed on the permanent record card, giving the LBC the athlete’s registration history along with his bout information. The back of the permanent record card is designed to record restriction information concerning the athlete. If the athlete receives a restriction at an event, that information is recorded both in the passbook and on the permanent record card. Again, if the athlete loses his passbook, his restriction information is not lost.
PASSBOOKS
The passbook belongs to the athlete and cannot be withheld from him or her except in the event of a restriction or formal disciplinary action.
Space is provided on the inside cover of the passbook for the athlete’s photo and signature. To avoid exchanging passbooks or photos, the photo should be firmly secured. 2” wide packaging tape is about the right size.
The first page contains the same information as the permanent record card, including space for validation labels. Once a validation label for the current year is placed in the passbook, the boxer may participate in competition.
The third page contains space for recording information regarding any restrictions issued to the athlete. On the following 17 pages of the passbook, the boxing record/history is recorded. Each time the athlete competes, the bout is recorded, including the date, event, and place of bout; the athlete’s weight; the results of the pre- and post-bout physicals by the ringside physician. Altered entries are void.
Per USA Boxing rules, all boxers must present their passbooks prior to weighing in. The results of every bout are recorded in the passbook. Besides the permanent record card, this is the boxer’s only record of his/her bouts. This information is not recorded at USA Boxing. Please don’t call USA Boxing to verify an athlete’s competition record.
Replacement of filled passbooks :
The passbook has space for seven validation labels. The eighth year a boxer registers for membership, a new passbook will need to be issued. The full passbook must be turned in to the registration chairperson and a new passbook will be issued as follows:
1. Transfer the information from the upper portion of the old book to the new book. Update information as necessary, such as new address or club affiliation, and certify the date of birth by initialing in the space provided.
2. Count the number of wins and losses recorded in the old book, add them to the number recorded on the first page (record of bouts at initiation of the passbook), and enter those numbers on the first page of the new book.
3. In the space in front of the validation labels, change year 1 through year 7 to year 8 through year 14.
4. On the front page of the passbook, under record of bouts at initiation of passbook, write Second Passbook Issued.
5. The athlete must provide new photographs and sign the inside cover of the new passbook.
Lost Passbooks (duplicates):
If the athlete has lost his passbook, he/she must complete a new membership application. The new application must be submitted with two new photos to the LBC Registration Chairperson, with a written statement signed by the athlete and his coach as to how, when, and where the passbook was lost and how many bouts the athlete has won and lost.
The LBC Registration Chairperson places a new validation label on the membership application, in the passbook and on the permanent record card. The information on the first page of the passbook is filled in as if it was a new book, except the birth date may be verified from the permanent record card, and the number of won and lost bouts must be recorded.
The words “Duplicate Passbook” should be written in the space below the win/loss record.
Mark “Duplicate” on the membership application and forward to USA Boxing.
Transfers:
When an athlete moves from one LBC to another, the new LBC handles the transfer. The athlete completes a new athlete membership application form and sends it with his/her passbook to the LBC registration chairperson in the athlete’s new LBC. The new LBC needs to contact the old LBC to request the athlete’s permanent record card be forwarded to
the new LBC. The old permanent record card will then be stapled to the new card, which the new LBC
registration chair will fill out. If the old permanent record card is not forwarded, the new LBC must verify the athlete’s birth date.
The new LBC registration chairperson validates the passbook, places a Transfer/Change label on the first page, over the old information, writes in the new information and returns the passbook to the athlete. Mark “transfer” on the athlete’s application, place a validation sticker on it and send to USA Boxing. If the
athlete has not already registered for the current year, the application must be accompanied by the registration fee.
NON-ATHLETE REGISTRATION
Non-athlete members, such as officials, coaches, clerks, physicians, master officials, etc., are registered on the non-athlete registration form. The form must be completely filled out and signed by the non-athlete.
Non-athletes must have reached their 18 th birthday.
Non-athlete members must complete a non-athlete registration form and sign the release and waiver. Non-athlete members must also consent to a background screening and complete the consent form with all the necessary information. (See below).
The A validation label is placed on the top copy to be sent to USA Boxing.
The B label is placed on the yellow copy and retained in the LBC files.
The C label is placed on the pink copy and returned to the non-athlete member as their proof of membership, except in the case of officials as noted below.
Background screening policy: In an effort to protect the youth and other participants of USA Boxing, and raise the standards of conduct within USA Boxing, a Sexual Abuse and Molestation Background Check is now mandatory for all “non-athlete” members of USA Boxing. These nonathlete members include coaches, officials and other volunteers.
A written authorization form allowing USA Boxing to order an individual Sexual Abuse and Molestation check will be the responsibility of the LBC to collect and forward to the National office. Failure of an individual to agree to this consent will result in immediate suspension from USA Boxing. Membership will be voided and any paid member fees, less the background screening fee, will be returned to the individual.
Individual applicants shall be disqualified as a member of USA Boxing if they have ever beenconvicted of or pled guilty or nolo contendre to the following:
1. Any crimes against children
2. Any sexual offense, regardless of the amount of time since the offense; examples include child molestation, rape, sexual assault, sexual battery, sodomy, prostitution, solicitation, indecent exposure, etc.
Should either of these charges be uncovered from the Background check, or should any of these charges be brought against an Applicant while a member of USA Boxing, the Applicant will be suspended from USA Boxing until such time as the charges have been cleared. Charges, which are dismissed due to insufficient evidence, or failure to continue prosecution, will be looked at on an individual basis.
The company conducting the Background Screening will notify the Applicant that a disqualifying entry was reported on the Background Check and present the Background Report and FCRA (Fair Credit Reporting Act) letter to the Applicant. In the event the Applicant feels a mistake has been reported in their report, it will be the Applicant’s responsibility to contact the reporting agency and resolve any issues.
All information obtained in the Background Screening is confidential information and will not be disclosed outside of the company conducting the Background Screening, except to USA Boxing, indicating that the Applicant did not pass the screening. USA Boxing is not responsible for any errors or omissions with regard to the Background Screening process.
The background screening will be conducted every other membership year. Non-athlete fees have been raised so that one half the fee is collected each year, effectively raising the non-athlete fees to $68.00 per year.
New Officials : When registering, new officials are issued the Officials Passbook. New officials must provide a photo, along with the completed form and membership fee. The photo and the C validation label will be placed in the passbook. The inside front cover of the passbook must be signed by the official. When the official becomes certified, the clinician will enter the certification information in the passbook.
Renewing Officials : When renewing, the official must present the Officials Passbook, a completed membership application and the membership fee. A new validation label will be placed in the passbook. USA Boxing, within about 30 days of receipt of the non-athlete membership form from the LBC, will mail direct to the non-athlete member a membership card, USA Boxing patch, and a membership letter. If the non-athlete member has been a certified coach or official, that information, including the level of certification, will be included on the membership card. Even if the member is no longer coaching or officiating, the certification information will still be on the card. USA Boxing will not delete the information from the computer, so it will be printed on the card.
PHYSICIANS: Physician members do not have to pay a fee for membership or for the background screening. Physicians are M.D.s or D.O.’s only. Nurse practitioners, EMTs, etc, may assist physicians at ringside but still must pay a membership fee. Any renewing physician will be automatically renewed by the National Office. The physician will need to consent to a current background screening every other year. New physicians will need to register with their LBC the first year of membership.
CLUB REGISTRATION
For an organization to qualify to register for a club membership, that club must have the same objectives as stated in USA Boxing’s Bylaws. Club membership begins on the day of registration and expires on December 31. Liability insurance coverage is only in place during the time period the club is registered. Renewing clubs are encouraged to register for the new year prior to January 1, so there is no lapse in coverage.
The club must complete the club membership application, sign it, and submit the application to the LBC with the membership fee. For 2010, the membership fee will be $175.00 to USA Boxing. Please encourage the clubs to provide a mailing address for the club, if it is different from the physical location of the training facility.
The LBC must verify that the club representative listed is a non-athlete member of USA Boxing. The club representative must be a current member and provide a validation number before the club application can be accepted. A club representative who is not a member cannot register their club, apply for a sanction or have voting privileges on the LBC Board of Governors.
The LBC registration chairperson should assign new clubs a club number. Club numbers should remain the same from year to year.
The LBC registration chairperson transmits a copy of the club application, both sides, to USA Boxing. If the club needs a Certificate of Liability Insurance, a certificate request should be filled out and submitted with the application, for 2010 there will be a $25.00 fee to process certificate of liability requests, and a $10.00 fee for any changes made to certificate of insurance already processed. USA Boxing does not automatically request certificates from the insurance company.

Sanctioning Policy
All sanction requests are to be submitted to the Chief of Officials, Richard Ellis, elrock98@yahoo.com for review and assignment. All members of the Virginia Association of USA Boxing that submit requests for a sanction, for amateur only contests, must do so no later than 30 days prior to the event.
All members requesting a Pro-Am sanction must submit there sanction no later than 60 days prior to the event.
The deadlines include full payment of the required sanction fee ($350).
Sanctions not submitted on time will not be approved unless authorized after consideration by the association board of directors and reserves the right to approve or disapprove all sanctions.
A fee of $50 will be charged for the use of the associations gloves for club events. Only registered clubs may use the gloves and the use is not authorized or allowed for Pro-Am events. Requests are to be submitted to the association president for consideration by the sanctioning deadline.
All fliers and posters for an event must state: "Sanctioned by the Virginia Association of USA Boxing." and the assigned sanction number.
USA BOXING SANCTIONED EVENT PROCEDURE
The Local Boxing Committee, within whose borders an event is being held, sanctions all amateurboxing competitions, except international and protected competition, which is sanctioned by USA Boxing. If state laws prohibit participation in an event according to USA Boxing rules, events cannot be sanctioned in those states.
When an event is sanctioned, the applicant agrees to abide by all USA Boxing rules and the LBC approves the applicant to hold an event. Only USA Boxing member clubs may apply for a sanction, and the club representative must be a non-athlete member. Only USA Boxing member athletes may
participate in the event. The LBC is responsible for making sure the event is conducted in ccordance with USA Boxing rules and applicable state laws.
Sanction fees are $350 for the first day and $50 for each day thereafter. The LBC may charge an amount up to and not exceeding three times the sanction fee, or an additional $750.00, to be retained by the LBC.
SANCTIONED EVENT PROCEDURES FOR BOXING CLUBS
How to apply for a sanction:
Application for a sanction is done in writing by completing the USA Boxing Sanction Application Form and submitting it to the LBC Chief of Officials, Richard Ellis. Sanctions cannot be issued after the event.
Select a date and site for your competition at least 30 days prior to the event.
1. Contact your LBC Chief of Officials, Richard Ellis to determine if the date is available and does not conflict with other boxing events. This is important to make sure there are enough officials available to conduct the event.
2. Request the Chief of Officials to send you a sanction packet containing all the necessary forms required to apply for the sanction. This should include a sanction application, certificate of liability request form, feature bout application and any other forms required by the LBC.
3. The sponsoring club should complete all forms and return them to the LBC as instructed by the LBC. The LBC Chief of Officials will not issue a sanction until all forms have been completed and the sanction fee has been paid. The LBC cannot issue a sanction over the phone.
4. The sanction is issued when the LBC Chief of Officals or designee has signed the sanction form and issued a sanction number, indicating approval of the event.
After your sanction has been approved, you will receive a bout packet containing all the forms necessary to conduct your event. The packet should include: bout packet instruction, official sanction (the application with the signature of the LBC Chief of Officals or designee becomes the
official sanction), entry form, report of boxing show, report of sanctioned event, financial statement for competition, instructions for head injury, treatment release forms, injury report forms, incident report, and sample ring set-up.
The Sanction shall be posted in a conspicuous place at the event, such as in the weigh-in room.
After the event:
Return all reports to the LBC registration chairperson within the required time frame.
These reports must be forwarded to the LBC within 72 hours:
Entry forms
Report of Boxing Show
Restrictions Affidavits* and applicable boxer’s passbooks
Treatment release forms*
Incident reports
*copies of these forms must also be sent to the LBC president

Sanction Requirements
Sanctions are issued, withheld or withdrawn in accordance with the following regulations:
1. No sanction will be issued by any LBC without first receiving a completed application for sanction form.
2. The requirements set forth in USA Boxing Constitution and Bylaws shall be complied with in accordance with the guidelines set forth in this Sanctioning Procedure Manual and other appropriate publications or the Constitution and Bylaws.
3. No sanction will be issued to a club or organization that is not a member of an LBC of USA Boxing.
4. No sanction will be issued to any organization whose interest in amateur boxing is purely commercial or where the event is to be promoted solely for the profit or the advertising value to be derived there from. Any income derived from sanctioned events must be used for the further promotion of amateur boxing, for an approved charity or for the general welfare of the promoting organization as a whole.
5. Sanctions issued to one organization cannot be transferred by that organization to another.
Any sanction so transferred shall be null and void for all purposes, and the LBC is empowered to deny further sanctions to any organization violating this provision.
6. No further sanction will be given to any organization which has failed to fulfill expense obligations to boxers or to give prizes as stated on its entry blank.
7. No sanction of any boxing competition will be granted for any event where the word “Olympic” or any derivative thereof is used in any advertisement or notice in any manner in connection with said boxing event unless written consent is obtained from the secretary of the Corporation. The terminology “Junior Olympic” can be used only in Junior Olympic Championships.
8. All sanctions must be signed by the Local Boxing Committee (LBC) president or his/her designee, and a record thereof must be maintained by that LBC.
Conditions of Sanction
All amateur boxing competitions of any kind where sanction is required according to USA Boxing
Constitution and By-Laws are subject to the following conditions:
1. The individual is registered as provided in USA Boxing Constitution and Bylaws.
2. Announcements and entry blanks of sanctioned events must state that no entries will be accepted unless the boxer is registered.
3. The registration number of each boxer must accompany the entry blank and his/her affiliation must be displayed before or after his/her name on the program.
4. Entry blanks, advertising and the program must bear conspicuously the statement: “held under the sanction of United States Amateur Boxing, Inc.” and include the address of the local boxing committee.
5. All organizations which have received a sanction for any contest where prizes are stated on entry blanks shall be required to send to comply with USA Boxing Rulebook, section 311.1.
Conditions for Withdrawing a Sanction (refer to USA Boxing Policy Manual) Every competition sanctioned by USA Boxing must comply with the requirements of USA Boxing. If an LBC believes an organization has not complied with the requirements of the above-mentioned policies, the sanction may be withdrawn under the following conditions.
1. The LBC must provide written notification outlining the problem of non-compliance. 2. If a hearing is requested, the complaint must be sent to the National Office in writing.
Financial Requirements for a Sanctioned Event
Requirements regarding finances include:
1. Each organization which has been granted a sanction hereunder shall within 10 days after holding the sanctioned event, file with the issuing local boxing committee (LBC) an itemized statement listing the following items. a. The name of each boxer and/or his representative to whom any payment of money or things of value were made. b. Registration number, affiliation and home LBC of each boxer to whom any payment was made. c. The amount of money paid to each boxer for transportation. d. The number of days the boxer was paid for meals and lodging. e. The separate amounts of money paid each boxer for meals and lodging. f. The signature of each boxer or his representative attesting to the amount of payment indicated as being correct. This statement shall be sent to the issuing registration chairperson and to the registration chairperson of the home LBC of each boxer to whom any payment was made.
2. The statement required shall be attached to and incorporated in an affidavit executed and sworn to on behalf of the organization by its responsible officer, manager or other representative having knowledge of the facts thereof on the form stipulated by USA Boxing.
3. The LBC may require from any other officer or person acting for the organization a similar affidavit and this affidavit shall be filed within 10 days after a written request.
4. The LBC may also require the organization to which the sanction has been granted to file a financial statement setting forth in detail all receipts and disbursements in connection with the sanctioned event, and such financial statement shall in such event be filed within 30 days after the holding of the event or within 10 days after a written request whichever shall be later.
5. The LBC may at any time require the organization to furnish, within 15 days after written request, all receipts and vouchers relating to any item or items relating to the sanctioned event.
6. In the event of failure or refusal to file with the Local Boxing Committee (LBC) any statement or affidavit required under any subdivision of this sanction, no sanction to hold any amateur boxing event shall thereafter be issued to the organization until such time as such statement or affidavit is filed, or until such time as the LBC may determine.
Pro-Am Boxing Policies and Rules
1) All currently registered USA Boxing member boxers are eligible to participate in USA Boxing-sanctioned Pro-Am competitions.
2) Pro-Am sanctions will only be granted to current USA Boxing member clubs, which have been registered for at least one year.
3) The following must be submitted to the LBC president or his/her designee in advance of any Pro-Am event as required by the LBC.
a. Completed USA Boxing Sanction Application form and sanction fee.
b. Certificate of 3 rd party liability insurance for the pro portion of the event.
c. Certificate of liability request for amateur portion of the event.
d. Certificate of accident insurance for pro boxers.
4) Upon receipt of the sanction application and related documents, it is the responsibility of the LBC president or his/her designee to check all paper work and information prior to signing and forwarding any Pro-Am sanction to USA Boxing’s national office.
5) Upon completion of the LBC president’s or his/her designee’s review and signing of the application, all paper work, must be sent to USA Boxing’s National office and received there not less than five (5) working days prior to the event. The national office will check all applications for completeness. Any incomplete or untimely applications will not be approved.
6) USA Boxing/the LBC will only be responsible for the amateur portion of the event. All of these events shall be governed by current USA Boxing rules, policies and procedures.
7) The LBC president or his/her designee must complete and send to USA Boxing’s national office a report of the Pro-Am event within 30 days following the event.
